An M3U playlist is simply a file listing the stream addresses of your live channels. Providers hand it to you — as a URL or a file — and your player reads it to populate your channel list.
M3U URL vs M3U file
A URL version is easiest: paste the link into your player and it always updates automatically. A downloaded file holds the same list but goes stale when the provider changes stream addresses.

VLC — open Network Stream and paste the URL. IPTV Smarters Pro — add a new user and choose M3U URL. TiviMate — add a playlist and select 'M3U Playlist'.
M3U vs Xtream Codes
Xtream Codes is a login-based method that unlocks channels plus the VOD and EPG from the provider's server. Many providers — including IPTV USA for new accounts — prefer Xtream Codes for new subscribers, with M3U available as an alternative.
